Colt Flake DDS CEO

Dr. Colt grew up in Snowflake, Arizona. He loved his childhood in a small town where he enjoyed playing football, basketball and golf. After high school he served a two-year mission in Mexico for the LDS church. He learned to speak Spanish fluently and gained a great love for the people there. While attending Arizona State University he met and later married his best friend and lovely wife, Ashley. Dr. Colt loves business, but has always known he wanted to be a dentist. After earning a business degree at ASU, he and Ashley moved to Colorado where he attended the University of Colorado School of Dental Medicine. In 2010 Dr. Colt graduated as a Doctor of Dental Surgery and began a two year specialty program at the University of Nevada Las Vegas to specialize in treating children. In 2012 Dr. Colt and
Ashley got lucky and had twin boys, Casen and Oliver. Then in 2019 they were fortunate enough to adopt their daughter Scottie from China. Dr. Colt loves spending time with his family outdoors, golfing, scuba diving, and traveling, but most of all he enjoys a nice piece of cheesecake!

Jeremy Cox DDS CCO

Dr. Jeremy Cox is an AAPD board-certified pediatric dentist living in Grand Junction Colorado. He is originally from Mesquite Nevada where after high school he served a two-year mission for his church in Rochester New York. Upon returning home, Dr. Cox attended college gaining a Bachelor’s degree from Southern Utah University. He attended dental school at the UNLV School of Dental Medicine in Las Vegas where he served as dental Student Body President and received his Doctor of Medical Dentistry degree. Dr. Cox then attended the University Of Nebraska Medical Center’s Pediatric Dental Residency program at Children’s Hospital in Omaha Nebraska for two years.

Dr. Cox then moved to Grand Junction in 2012 and founded Pediatric Dental Specialists of Western Colorado. Dr. Cox is a Diplomate of the American Board of Pediatric Dentistry and a board-certified pediatric dentist which is the highest credentials in the field of Pediatric Dentistry. Dr. Cox enjoys teaching and educating other dentists, pediatricians, and other medical professionals on pediatric dentistry. Dr. Cox enjoys medical mission trips providing dental care to children in need. He has provided dental care to children in Mexico, Dominican Republic, Haiti, and others. He donates his time and serves on the Quest Cleft Palate Team of Grand Junction. Dr. Cox strives to create an office where the staff are cared for and respected, and our patients receive the highest level of dental care while learning to love the dentist.

Scott Crews CDO

Scott Crews began in the healthcare industry in the late 80s and early 90s. He was CFO of a regional hospital management company. Scott joined American Dental Partners in 1996, building University Dental with Dr. Ray Garrison and grew it to $27,000,000 over 8 years. In 2008, Scott Crews and Clark Caflisch founded National Dental Development Premier Services. NDDPS performs services to the DSO industry such as building an acquisition pipeline, integrate practices into the buyer’s network, advise on organization structure and financing, advise on entity structure for group practice, and building scalable RCM teams. NDDPS has several private equity relationships and performs services for them as listed above in addition to locating platform DSOs to invest in.

Clark Caflisch, COO

Clark has several decades of expertise specializing in enterprise dental infrastructure, business process, Revenue Cycle Management, analytics and outsourcing/offshoring. He was a founder/partner to developing two enterprise practice management systems that support large-scale DSOs operations.
